SHAH ALAM, Aug 3 — The Selangor Occupational Safety and Health Department (DOSH) has issued a prohibition notice at a factory construction site in Kuala Langat following a fatal accident on Tuesday (July 28) that claimed the lives of two foreign workers.
Harian Metro reported that its Director's Office said the notice was issued after an inspection and preliminary investigation revealed several failures relating to occupational safety and health at the construction site.
Among the notices issued were those for failure to carry out a risk assessment for concrete works involving a mobile crane, failure to prepare a lifting plan for crane operations, and failure to appoint a site safety supervisor.
In the 5pm incident, two general workers aged 40 and 44 died after being crushed by the structure of an overturned crane.
Selangor DOSH added that the accident happened while workers were lowering a concrete bucket weighing an estimated 400kg using a mobile crane.
"The crane is believed to have overturned because the boom had been extended beyond the appropriate operating length," it said in a statement.
Both victims were beneath the boom at the time of the incident before they were struck by the collapsing structure.
The two men were pronounced dead at the scene after suffering severe head injuries when they became trapped between a beam structure and the fly jib boom. Their bodies were later taken to the Banting Hospital for post-mortem examinations.
Following the incident, a team of investigating officers was dispatched to the site to conduct further inquiries. A technical inspection found physical damage to the crane's fly jib boom structure.
Selangor DOSH noted that further investigations also revealed that the project site was not registered with DOSH. In addition, both the main contractor and the subcontractor failed to produce any documents relating to the management of occupational safety and health at the construction site.
The case is being investigated under Section 15(1) of the Occupational Safety and Health Act 1994.
